The Plymouth Patriot Award is given to students at the end of the year who have completed a set of criteria. Below you will see what each grade level must do to qualify for this award.
All students have been asked to memorize the following: Last name, telephone number & address.
Kindergarten Requirements
Student must complete 13 items.
1. Have 95% attendance –no more than 8 absences.
2. Have no more than 8 tardies.
3. Print first name correctly.
4. Recognize and draw 5 basic shapes. ( triangle, square, rectangle, circle, hexagon)
5. Write numbers 0-20 correctly.
6. Recognize numbers to 30.
7. Name all capital letters.
8. Name all lower case letters.
9. Name all letter sounds.
10. Read 25 sight with 80% accuracy.
11. Solve addition and subtraction problems, with objects, to 10.
12. Count to 100.
13. Recognize ordinal numbers, in order, 1st to 5th
14. Recite days of the week in order.
15. Recite months of the year in order.
16. Know the first sound of given words.
17. No white slips.
First Grade Requirements
Students must complete 15 items.
1. Have 95% attendance-no more than 8 absences.
2. Have no more than 5 tardies.
3. Maintain good citizenship-no white slips.
4. Have only 4’s and 5’s in citizenship all year.
5. Write first and last name.
6. Read 100 basic sight words with 90% accuracy.
7. Have only 4’s and 5’s in homework completion for the entire year.
8. Read 40 words per minute on DIBELS fluency test.
9. Score 5 or more on YPP reading assessment 5 times.
10. Demonstrate growth in reading and math or be on grade level.
11. Earn 100% on 10 spelling tests.
12. Write numbers 0-100.
13. Count by 1’s to 100 and by 2’s to 20.
14. Count by 5’s and 10’s to 100.
15. Count like coins up to $0.25 (pennies, nickels, or dimes).
16. Score 20 or higher on YPP math assessment 8 times.
17. Achieve 90% on five math concept tests.
18. Do a report.
19. Participate in the reflections contest
20. Parent and child attend all 3 SEP Conferences.
Second Grade Patriot Award Requirements
Student must complete 12 of 18 items by Friday, May 6, 2016.
*4 Required: *
1. Know first and last name, phone number and address.
*2. Have 95% attendance – no more than 7 absences.
*3. Have no more than 5 tardies during the year.
*4. Be able to log into the computer on your own.
5. Maintain good citizenship – no visits to Mr. Graham.
6. Write 1 original story at home and present it to the class.**
7. Use the symbols greater than, less than and equal to correctly.
8. Read 200 high frequency words in 3 minutes.
9. Write numbers 0 – 1,000.
10. Determine the value of any 5 coins up to a dollar.
11. Earn 80% on 10 Reading Tests.
12. Achieve 80% on at least 5 math chapter tests.
13. Return 7 out of 9 homework reading calendars.
14. Read 90 words per minute on DIBELS fluency test by the end of the year.
15. Participate in the Reflections contest project.**
16. Parent and child will attend both (2) SEP Conferences.
17. Complete all addition and subtraction facts by the end of the year.
18. Skip count by 2’s to 100; Skip count by 5’s, 10’s and 100’s to 1,000.

Third Grade Requirements
Students must complete 12 items.
1. Have 95% attendance-no more than 8 absences.
2. Attain multiplication mastery through 10’s.
3. Maintain good citizenship-no white slips.
4. Score 20 or higher on YPP Language Arts test.
5. Score 24 or higher on YPP Math test.
6. 100% Work Completion.
7. 100% on 10 spelling tests.
8. Label 6 countries in South America.
9. Label the continents and oceans of the world.
10. Sign first and last name in cursive.
11. Achieve 80% or above on Math Benchmark Assessment.
12. Read 5 out of 7 days and have parents sign in planner or homework sheet.
13. Read 110 words per minute on DIBELS fluency test.
14. Participate in the Reflections contest.
15. Attend all 3 SEP conferences.
16. Memorize and present a poem or book report to the class.
17. Set a personal goal and work for at least 30 days to accomplish it.
4th Grade Patriot Award Requirements
Student must complete 3 required and 10 of 13 items
1. Know your full name in writing and speaking. (Required)
2. Know your address where you live in writing and speaking (number, street name, city and zip code). (Required)
3. Know your phone number with area code in writing and speaking. (Required)
4. Attend all 2 SEP Conferences during the school year.
5. Participate in the Reflections Contest.
6. Have 95% attendance – no more than 9 absences.
7. Maintain good citizenship – No Infraction Violation Forms all year.
8. Read 115 words per minute on DIBELS fluency test.
9. Improve DIBELS fluency test by 20 words by end of year. +
10. Turn in 7 required book reports on due date given by classroom teacher.
11. Create a model of a Native American home at home and turn in on due date given by classroom teacher.
12. Complete Utah County report projects at school and turn in by due date given by classroom teacher.
13. Write a final draft for 3 original stories and turn in by due date given by classroom teacher.
14. Complete a science fair project and turn in by due date given by classroom teacher.
15. Give one oral presentation by due date given by classroom teacher.
16. Turn in 8 required “Kindness Counts” monthly home projects by due date given by classroom teacher.
5th Grade Patriot Award Requirements
Student must complete 15 of 20 items
(Resource Students must complete 10 of 20 items)
1. Have 98% attendance-no more than 3 absences.
2. Less than 4 tardies.
3. Maintain good citizenship with no office referrals
4. Participate in History Fair.
5. Participate in Safety Patrol, or Recycling Team
6. Complete 6 or more book reports.
7. Complete a volcano project.
8. Pass off multiplication facts
9. Complete all monthly reading goals at home (30 minutes a day)
10. No zeroes throughout the year
11. Memorize and recite the “Parts of Speech” song.
12. Participate in math fair
13. Complete a science fair project
14. Read 124 words per minute on DIBELS fluency test.
15. Participate in the Reflections contest.
16. Memorize and recite a 120 word poem.
17. Complete a PowerPoint presentation on a social studies topic.
18. Student knows their home address, phone number, and their 9-digit student number
19. Earn 100% on Map test of the 50 states.
20. Attend 2 SEP conferences.
Sixth Grade Requirements
Students must complete 15 items.
1. Have 98% attendance-no more than 3 absences.
2. Participate in Safety Patrol.
3. Maintain good citizenship-no white slips.
4. Score 20 or higher on YPP Language Arts test.
5. Score 24 or higher on YPP Math test.
6. Maintain a B in each subject.
7. Have no office referrals all year.
8. Participate in Band or Orchestra.
9. Complete a Book share every month.
10. No missing assignments all year.
11. Fill out and have planner signed daily.
12. Excellent participation in grade level activities.
13. Complete a science fair project
14. Read 124 words per minute on DIBELS fluency test.
15. Participate in the Reflections contest.
16. Have no tardies.
17. Complete all monthly reading goals.
18. Pass 20 AR tests.
19. Complete a service project.
20. Attend all 3 SEP conferences.
