Please contact Mrs. Erickson to speak with her about any attendance or registration concerns you may have.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Attendance Information<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>To excuse a student&#8217;s absence:\n<ul class=\"wp-block-list\">\n<li>Call the school: 385-646-5125<\/li>\n\n\n\n<li>Email Mrs. Erickson: <a href=\"mailto:mwerickson@graniteschools.org\">mwerickson@graniteschools.org<\/a><\/li>\n\n\n\n<li>Written note to be turned in to the attendance office <strong>within 2 weeks<\/strong> of the absence to be excused.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>What to include when reporting a student absence:\n<ul class=\"wp-block-list\">\n<li>Student&#8217;s full first and last name<\/li>\n\n\n\n<li>Student number (if you know it)<\/li>\n\n\n\n<li>Date(s) of absence<\/li>\n\n\n\n<li>Reason for the absence<\/li>\n\n\n\n<li>Parent signature<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Missed periods are accrued into days of absence.&nbsp;<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">What to do when my student arrives late or must leave school early?<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Students arriving late<\/strong> can be excused when accompanied by a parent or with a written note signed by the parent to excuse the tardiness.&nbsp;<\/li>\n\n\n\n<li><strong>Students leaving the school<\/strong> for a portion of the day or for the rest of the day, must be checked out in the Attendance Office by a parent\/legal guardian<strong>. Identification is required.<\/strong> &nbsp;Students will only be released to people listed in Focus.<\/li>\n\n\n\n<li><strong><span style=\"font-size: revert; color: initial;\">Street Pass<\/span><\/strong><span style=\"font-size: revert; color: initial;\"><strong>&nbsp;<\/strong>may be issued for a student to leave the school early or for an appointment without a parent.&nbsp; To receive a <\/span><strong style=\"font-size: revert; color: initial;\"><em>street pass<\/em><\/strong><span style=\"font-size: revert; color: initial;\">, the student will need a note from a parent\/guardian listing the following:&nbsp;<\/span>\n<ul class=\"wp-block-list\">\n<li><span style=\"font-size: revert; color: initial;\">First and last name of the student<\/span><\/li>\n\n\n\n<li><span style=\"font-size: revert; color: initial;\">Student number<\/span><\/li>\n\n\n\n<li><span style=\"font-size: revert; color: initial;\">Date and time of check out<\/span><\/li>\n\n\n\n<li><span style=\"font-size: revert; color: initial;\">Reason for checkout<\/span><\/li>\n\n\n\n<li><span style=\"font-size: revert; color: initial;\">Parent signature and phone number in case of questions.&nbsp; <\/span>\n<ul class=\"wp-block-list\">\n<li><span style=\"font-size: revert; color: initial;\">The student will bring this note to the Attendance Office before school starts and it will be exchanged for a <\/span><strong style=\"font-size: revert; color: initial;\"><em>street pass <\/em><\/strong><span style=\"font-size: revert; color: initial;\">note<\/span><em style=\"font-size: revert; color: initial;\">.<\/em><span style=\"font-size: revert; color: initial;\">&nbsp; <\/span><\/li>\n\n\n\n<li><span style=\"font-size: revert; color: initial;\">The student will show this note to the teacher of the period when he\/she will be leaving.&nbsp; <\/span><\/li>\n\n\n\n<li><span style=\"font-size: revert; color: initial;\">The student can leave at the time listed on the note.&nbsp; <\/span><strong style=\"font-size: revert; color: initial;\"><em>Street pass<\/em><\/strong><span style=\"font-size: revert; color: initial;\"> notes will not be issued over the phone.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">How can I review my student&#8217;s attendance?<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/www.graniteschools.org\/infosystems\/parent-portal-resources\/parent-portal-attendance-web-version\/\">Parent Portal Instructions for reviewing student Attendance<\/a><\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">What if our family has a vacation scheduled?<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A parent may excuse up to 10 absences due to a family vacation prior to the vacation. If your student will be absent more than 10 days, please discuss options with Mrs. Erickson in the Attendance Office. Students are responsible for all school work they miss during their absences.<\/li>\n\n\n\n<li>Attendance Office Phone Number is (385) 646 &#8211; 5125<\/li>\n\n\n\n<li><a href=\"https:\/\/drive.google.com\/file\/d\/1T321tGon_XBiqYVYbcoe4ZZf8mJRSQG5\/view?usp=sharing\">Print Vacation Release Form (PDF)<\/a> and return to Attendance Office at your earliest convenience<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">To excuse absences during a vacation, please do the following:<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Obtain a Vacation Form from the attendance office.<\/li>\n\n\n\n<li>Complete the required portion of the form<\/li>\n\n\n\n<li>Student will then get the form signed by each of their teachers<\/li>\n\n\n\n<li>The completed for must be turned into the Attendance Office before leaving on vacation.<\/li>\n<\/ul>\n\n\n\n<p><strong>Granite School District Attendance Policies and Procedures<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/www.graniteschools.org\/schoolleadership\/school-registration-information\/attendance-procedures\/\">Attendance Policies and Procedures Language Translations<\/a><\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Student Attendance \/ Chronic Absentee Prevention Procedure Moratorium Guidelines<\/h4>\n\n\n\n<p>In keeping with our commitment to providing a quality education, schools are concerned when a student misses school for any reason. The mission of Granite School District is to prepare every student with the knowledge and skills needed for lifelong success in a changing world. Students who are excessively absent from school are not able to accomplish their educational goals. According to the Utah Compulsory Attendance Laws (Utah Code \u00a753G\u20136-201 et seq.), parents\/guardians and students are responsible for regular school attendance. Occasionally, a student must be absent from school for acceptable reasons such as illness, medical appointments, family emergencies, pre-approved vacation leave, etc. Please call or send a note explaining the reason for your student\u2019s absence to your school\u2019s attendance secretary to excuse the absence.<\/p>\n\n\n\n<p>In accordance with state law (Utah Code \u00a7 \u00a7 53G-6-202 and 203), school administrators, school resource officers, and absentee specialists may issue an Attendance Inquiry to parents\/guardians of a student who is absent at least five times during the school year. An Attendance Inquiry directs parents\/guardians to contact school support personnel to discuss attendance issues.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">District attendance procedures are as follows:<\/h4>\n\n\n\n<p>\u00b7 Excusal Notes must be received within ten (10) school days following the return from an absence, the school may deny excusal notes that are received after that point in time.<\/p>\n\n\n\n<p>\u00b7 After five (5) absent days within a school year, the school will mail or email the student\/parent\/guardian the First Attendance Inquiry letter notifying the student\/parent\/guardian of the attendance policy.<\/p>\n\n\n\n<p>\u00b7 After five (5) unexcused days within a school year, the school will mail the student\/parent\/guardian the Second Attendance Inquiry letter asking for the parent(s)\/guardian(s) help with their students attendance.<\/p>\n\n\n\n<p>\u00b7 After ten (10) unexcused days of being absent within a school year, the school will mail the student\/parent\/guardian a Third Attendance Inquiry letter inviting the family to attend a Parent Attendance Workshop hosted by a school representative.<\/p>\n\n\n\n<p>\u00b7 After fifteen (15) unexcused days of being absent within a school year, the school will mail the student\/parent\/guardian a Fourth Attendance Inquiry letter inviting the family to attend an Individual Attendance Support Meeting to implement a Student Attendance Plan.<\/p>\n\n\n\n<p>\u00b7 For secondary schools, missed periods will accrue into days of absences.<\/p>\n\n\n\n<p>Please check with the school attendance specialist\/secretary concerning excused or unexcused absences of your student. Your cooperation is needed in offering your student a quality education.
