To avoid collections: Fees must be paid within ninety (90) days of your student starting school. If applying for a fee waiver, a parent or guardian must submit application and documentation within thirty (30) school days.
Fee Waiver Applications must be completed annually for each student. Fee Waivers for the coming school year are accepted after July 18th.
The 2005 legislation (House Bill 183) requires all applicants for Fee Waiver to provide the school with documentation of eligibility upon application. Please mail or drop off your completed application (both pages) along with a copy of the necessary documentation for eligibility (please come prepared as the office staff are not able to make copies). We will not have in-person fee waiver application. Documentation should include the first page of your 2017 Income Tax Returns OR current verification of SSI, AFDC, Foster Care or State Custody. We must have proper documentation before your application will be processed.
Mail the following items to Eisenhower Jr. High School (or drop off after August 6th):
- Fee Waiver Application Form English or Spanish (Please make sure to fill out BOTH pages and provide a separate application form for each student.)
- 2017 Income Tax Return – COPY ONLY PLEASE! (We do not need the entire return – only the page listing your student as a dependent and showing Adjusted Annual Income – Sample Page) OR Current SSI, AFDC, Foster Care or State Custody verification
All of the documentation will be kept private during verification and then will be shredded. Please remember to send copies only. If the fee waiver is approved, your student’s schedule and locker assignment will be mailed. We will assign the best locker location available at the time the Fee Waiver is approved. If there is a problem, or we need more information, a denial letter will be mailed.
