Applying for a Fee Waiver<\/strong><\/h2>\n \n \n \n

To avoid collections: Fees must be paid within ninety (90) days of your student starting school. If applying for a fee waiver, a parent or guardian must submit application and documentation within thirty (30) school days.<\/strong><\/p>\n \n \n \n

Fee Waiver Applications must be completed annually for each student. Fee Waivers for the coming school year are only accepted after July 26th.<\/p>\n \n \n \n

Fee Waiver forms and documentation of income may be submitted electronically through the parent portal. The Registration tab will be available on the portal after July 26th.<\/em> <\/p>\n \n \n \n

You can also PRINT and MAIL the following items to Eisenhower Jr High School, 4351 S. Redwood Road, Taylorsville, UT 84123 (or drop off after July 26th)<\/strong>:  <\/p>\n \n \n \n

  1. Fee Waiver Application Form (Please make sure to fill out BOTH pages if required and provide a separate application form for each student.) <\/li>\n \n \n \n
  2. Documentation of eligibility (COPY of 2021 Income Tax Return \u2013 We do not need the entire return \u2013 only the page listing your student as a dependent and showing Adjusted Annual Income OR<\/strong> Current TANF\/FAP, SSI, Foster Care or State Custody verification.) PLEASE SEND COPIES ONLY as documentation will NOT be returned. <\/li>\n <\/ol>\n \n \n \n

    If the Fee Waiver is approved, your student\u2019s schedule will be sent to the email<\/strong> address we have on file after August 2nd.<\/strong>  If there is a problem with your application, or we need more information, we will attempt to call you and a denial letter will be mailed.<\/p>\n \n \n \n

    Thank you for your help with this process. We hope to make it as simple as possible to facilitate your Fee Waiver request.<\/p>\n \n \n \n

    If you have any questions, contact the administration at Eisenhower Jr. High 385-646-5154. It is your responsibility to verify that your Fee Waiver application has been received and processed.<\/p>\n \n \n \n
