This is mandatory BEFORE your student can be enrolled at Monroe Elementary.<\/p>\n<ul>\n<li>Verify physical house\/appt address is within Monroe boundaries<\/li>\n<li>Proof of Address dated within 60 days (Utilities bill, Mortgage statement, or Lease\/Rent Agreement) ***form to complete if this item not available<\/li>\n<li>Complete and sign front and back of registration form and school policies and disclosures<\/li>\n<li>Student Birth Certificate<\/li>\n<li>Immunization Record<\/li>\n<li>Parent or Guardian photo ID<\/li>\n<\/ul>\n<p><strong>Monroe Inscriptcon en Kinder<\/strong><\/p>\n<p>Por favor, aseg\u00farese de tener todos los documentos que se indica a continuaci\u00f3n y la tarjeta de registro completa.
