map to verify that your address is within our school boundary before continuing with registration.<\/strong><\/p>\n\n\n\n<p>As a new student to the West Lake boundary, you will need to complete the following:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Sign up for an appointment time to meet with our school registrar using this <a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSfLrYaWf5Tj4_tp3HyDExs6FBXv7UxMHgTaHAUl7oORro8_Xg\/viewform?usp=dialog\" target=\"_blank\" rel=\"noreferrer noopener\">Google Form<\/a><\/li>\n\n\n\n<li>Make your course selections (<a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSfmP9aAnEvz4bUCuGEnojOeBO6tzyFJnnm5s1C8xHi5UylAXA\/viewform?usp=dialog\" target=\"_blank\" rel=\"noreferrer noopener\">6th Grade (Google Form)<\/a>) (<a registration<\/a><\/li>\n<\/ol>\n\n\n\n<p>To complete your registration, you <strong>must bring<\/strong> the following documents to the school (3400 South and 3450 West) at your appointment time:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Proof of Residence<\/li>\n\n\n\n<li>Birth Certificate<\/li>\n\n\n\n<li>Immunization Records\n<ul class=\"wp-block-list\">\n<li>A current utility bill or lease agreement establishing your&nbsp;<b>proof of residence<\/b> within the school boundaries (not required for those attending on an open-enrollment permit).*<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p><span style=\"font-size: small;\">*If the proof of residence is not in the name of the legal guardian of the student, you must complete an
