You may fill out the registration card online (below), but you will also need to make an appointment to bring certain documents to the school to complete your registration. These documents include:
- A current utility bill or lease agreement establishing your proof of residence within the school boundaries (not required for those attending on open-enrollment permit).*
- Birth certificate accompanied with parent/guardian ID
- Immunization records
If you don’t have one of the above mentioned items, please still fill out the registration card below, and bring the necessary documents as soon as possible. Your student’s registration will not be complete until we receive them.

*If the proof of residence is not in the name of the legal guardian of the student, you must complete the “Affirmation of Residence” form with a notary.
