Enrollment Information (All New Students to Cottonwood)

If your child is new to Cottonwood this year and you have not visited with our office staff to enroll, you must sign up for an enrollment appointment:

Required Registration Items:

• Enrollment-Form (PDF)
• Immunizations Form 
  For a list of required immunizations, please visit the Utah Health Department’s Website (URL)
• Original Birth Certificate (copies will not be accepted)
• Proof of Residence (Current gas/electric bill within last 60 days; lease agreement; mortgage statement)
• GSD Online Registration Information (URL) (The online portion of registration will open TBD)
