If your child is new to Cottonwood, or you wish to attend, and you have not visited with our office staff to enroll, please call us for an appointment, 385-646-4798 or come to our main office between 8:00-3:00 on school days.
If you are wishing to enroll and live outside of our school boundaries (PDF), please fill out a School Choice Application found on Granite School District’s website.

For a list of required immunizations, please visit the Utah Health Department’s Website (URL) - Original Birth Certificate (copies will not be accepted)
- Proof of Residence (Current gas/electric bill within last 60 days; lease agreement; mortgage statement)
- GSD Online Registration Information (URL) (The online portion of registration will open TBD)
