This page goes over medical forms you may need to prep for the upcoming school year.

💊 If your student needs medication(s) stored at the front office, please fill out the ‘Medication Authorization Form’ and return it to the front office next year.
🥜😤 Students with allergies and asthma can just have parents and their provider fill out the ‘Allergy Emergency Action Plan’ and/or the ‘Asthma Emergency Action Plan’ for things like epipens, benadryl and inhalers. All other medications will still need the ‘Medication Authorization Form’.
🍔 If your student has food allergies, please fill out the ‘Special Meals Accommodation Form’ and return it to the front office for special meal accommodations next year.
