To avoid collections: Fees must be paid within ninety (90) days of your student starting school. If applying for a fee waiver, a parent or guardian must submit application and documentation within thirty (30) school days.

Fee Waiver Applications must be completed annually for each student. Fee Waivers for the coming school year are only accepted after July 26th.

Fee Waiver forms and documentation of income may be submitted electronically through the parent portal. *The Registration tab will be available on the portal after July 26th.* 

You can also PRINT and MAIL the following items to Bonneville Jr High, 5330 S. Gurene Dr, Holladay, UT 84117 (or drop off after July 26th):  

  1. Fee Waiver Application Form (Please make sure to fill out BOTH pages if required and provide a separate application form for each student.) 
    • Fee Waiver – English (PDF)  
    • Fee Waiver – Spanish (PDF)  
    • Fee Waiver – Arabic (PDF)  
    • Fee Waiver – Chinese (PDF)  
    • Fee Waiver – Somali (PDF) 
  2. Documentation of eligibility (COPY of 2021 Income Tax Return – We do not need the entire return – only the page listing your student as a dependent and showing Adjusted Annual Income OR Current TANF/FAP, SSI, Foster Care or State Custody verification.) PLEASE SEND COPIES ONLY as documentation will NOT be returned. 

We must have the proper documentation before your application will be processed. All of the documentation will be kept private during verification. If dropping off your application, please remember to come prepared with copies of your documentation. Office staff are not able to make copies.

If the Fee Waiver is approved, your student’s schedule will be sent to the email address we have on file after August 2nd.  If there is a problem with your application, or we need more information, we will attempt to call you and a denial letter will be mailed.

Thank you for your help with this process. We hope to make it as simple as possible to facilitate your Fee Waiver request.

If you have any questions, contact the administration at Bonneville Jr High at 385-646-5124. It is your responsibility to verify that your Fee Waiver application has been received and processed.

**REMINDER: Fee Waivers and Free/Reduced Lunch programs are separate and are not available until the fall each year. You can apply for Free/Reduced lunch online at www.myschoolapps.com after August 1st.
