To register a new student to Eisenhower:
1. Please use the School Search Map to verify that your address is within our school boundary
2. Please call our Registrar 385-646-5155 to make an appointment
3. For your appointment you will need to provide physical copies of:
- Student’s birth certificate
- Up to date immunization record
- Proof of address with legal parent/guardian’s name on it
- Accepted Proof of Address verification – Rental Agreement or Utility Bill
- Driver’s License will not be accepted
- Name and address of previous school
- Checkout papers from previous school
