Guest Requirements for the KHS May 28th, 2021 Senior Ball

• Guests must complete a Guest Dance Permission Form.
  1. The form can be picked up in the Main Office, Monday – Friday, 7:00am – 2:30pm.
  1. No forms will be available after 10:00am, May 28^th, 2021.
  1. If the guest is from another high school, they must visit with a KHS administrator to complete the Guest Dance Permission Form.
  1. The completed form must be submitted when purchasing the ticket.

• Guests must provide verifiable proof of a “Negative Result” from a standard COVID19 Test, at the time of the ticket purchase.
  • The ticket must be purchased within 48 hours of the Negative COVID19 Test.
  • Guests cannot COVID19 test at Kearns High School.
  • The guest may only purchase 1 ticket.

• Guests must attend high school or be an early graduate with the Class of 2021.

• Guests cannot be currently suspended or expelled from the school they attend.

• Guests tickets are available from the bookkeeper on:
  • May 27^th
    • 7:00am-7:30am
    • 10:33am-11:08am
    • 12:04pm-12:39pm
    • 2:10pm-3:30pm
  • May 28^th
    • 8:00am-12:00pm
  • NO TICKETS SOLD AT THE DOOR!

• Guests must abide by all KHS rules & regulations.
