Morningside Elementary New Student Enrollment
New student enrollment is for students who are new to Morningside Elementary, or who are returning after attending another school.
If your child will be entering kindergarten for the first time, please use the Kindergarten Enrollment form.
New student enrollment is comprised of three steps:
- Verify that you live within Morningside Elementary’s boundary
- If you live outside of the boundary you must apply for Open Enrollment. Once the application has been approved you may proceed with enrollment. We will not enroll out-of-boundary students without an approved open enrollment.
- Complete the new student enrollment form (see below)
- Provide the required enrollment documents.
Your registration will NOT be complete until you have provided copies of the following documents:
- Proof of Residency
- Must be in the name of the student’s legal guardian, please see the expandable section below for a list of qualifying documents.
- Student’s Original Birth Certificate
- Student’s Immunization Record (Must contain the student’s name)
- Guardian Identification (Driver’s License, Utah Identification Card, Military ID, or Passport)
Qualifying Proof of Residency Documents (click to expand)
- Copy of deed AND/OR record of most recent mortgage payment; or
- Copy of Lease AND record of most recent rent payment; or
- Legal affidavit from landlord affirming tenancy AND record of most recent rent payment of available, or statement in body of affidavit that family is living there free of charge and for what period of time (the school can provide you with a copy of this form to take to a notary);
- If none of the above is available, a legal affidavit from the leader of a recognized established religious unit (ward, stake, parish, mosque, synagogue) affirming that the family lives at the address designated by the parent/legal guardian.
- Letter from a government agency
- Payroll stub
- Bank or credit card statement
- Utility bill or work order such as:
- Gas bill
- Electric bill
- Home telephone or cell phone bill
- Cable bill
- Change of address form for U.S. mail
- Valid driver’s lisence
- Current vehicle registration
- Valid Utah photo identification card
- Valid passport
- Voter registration documentation
- W-2 form (dated within the past year)
- Property tax bill (dated within the past year)
The document must show the current address and, unless otherwise noted, be dated within the past 60 days. If you are unable to provide any of these documents, please speak with the school.
If you are registering an ALC student, you must have completed the following:
- Tested for ALC using the CogAT which can be done here at Morningside or at the district by appointment
- Received an invitation to the program from Tamara Wright at the district
