Students new to Granite School District, incoming preschool and kindergarten students must provide the following information at the time of registration:

Completed Health Profile

Original Birth Certificate

Copy of your child’s Immunization record

Proof of Address (recent copy of electricity, gas, or water bill in parent’s name, or rental agreement or mortgage statement in parent’s name—NO OTHER PROOF WILL BE ACCEPTED.)
Incoming kindergarten students must be five years old on or before September 1st to be eligible.
