APPLYING FOR A FEE WAIVER
To avoid collections: Fees must be paid within ninety (90) days of your student starting school. If applying for a fee waiver, a parent or guardian must submit application and documentation within thirty (30) school days.
Fee Waiver Applications must be completed annually for each student. Fee Waivers for the coming school year are only accepted after July 26th.
Fee Waiver forms and documentation of income may be submitted electronically through the parent portal. *The Registration tab will be available on the portal after July 26th.*
You can also PRINT and MAIL the following items to Eisenhower Jr High School, 4351 S. Redwood Road, Taylorsville, UT 84123 (or drop off after July 26th):
- Fee Waiver Application Form (Please make sure to fill out BOTH pages if required and provide a separate application form for each student.)
- Documentation of eligibility (COPY of 2021 Income Tax Return – We do not need the entire return – only the page listing your student as a dependent and showing Adjusted Annual Income OR Current TANF/FAP, SSI, Foster Care or State Custody verification.) PLEASE SEND COPIES ONLY as documentation will NOT be returned.
If the Fee Waiver is approved, your student’s schedule will be sent to the email address we have on file after August 2nd. If there is a problem with your application, or we need more information, we will attempt to call you and a denial letter will be mailed.

**REMINDER: Fee Waivers and Free/Reduced Lunch programs are separate and are not available until the fall each year. You can apply for Free/Reduced lunch online at www.myschoolapps.com after August 1st.
