Sign up for an appointment time to meet with a counselor by calling 385-646-5196. Please call for an appointment starting July 27th.
To complete your registration, you must bring the following documents to the school (5850 South 5600 West) at your appointment time:
- Proof of Residence
- Birth Certificate
- Immunization Records
- A current utility bill or lease agreement establishing your proof of residence within the school boundaries (not required for those attending on an open-enrollment permit).*
*If the proof of residence is not in the name of the legal guardian of the student, you must complete an “Affirmation of Residence (PDF)” form with a notary.

- Birth certificate accompanied with parent/guardian ID

- Immunization records complete through 7th grade as outlined in the Department of Health’s requirements for 7th grade (PDF) (Español (PDF))
Unfortunately, your student’s registration will not be complete until we receive these documents, and we cannot give your student a schedule until the registration is complete.
