Utah Immunization Rules for Students Link
To access your child’s immunization record or locate a near by clinic, please visit the Utah Immunization Program website at https://immunize.utah.gov
All copies of immunization or exemption forms can be dropped off at the school beginning July 21st, faxed to the school at 385-646-5127, or emailed to mwerickson@graniteschools.org. Please submit a copy of the record by the first day of school.
Additional Immunizations are REQUIRED for 7th Grade Enrollment
This message is a reminder for all 7th grade students and parents. To attend 7th through 12th grade, a student must provide written proof of receiving the following immunizations:
· 1 dose of Tdap
· 2 doses of Varicella (chickenpox)
· 1 dose of Meningococcal Conjugate
· 3 doses of Hepatitis B
· 2 doses of Measles, Mumps, Rubella (MMR)
· 4 doses of Polio
· 2 doses of Hepatitis A
· Proof of receiving 5 doses of DTaP, or if never or not fully vaccinated, proof of receiving the appropriate number of Td or Tdap doses based on the student’s age.
If your student has received all these vaccines, please submit a written copy of the record to the school by the first day of classes.
If your student has not received these immunizations, please make an appointment with your healthcare provider, pharmacy, or local health department to receive them and submit a copy of the record to the school before the first day of classes.
For those who choose to exempt their children from all or some of the required vaccines, or if your child already has an existing exemption form, please ensure that the exemption form you have clearly shows all the vaccines from which your child is exempt.
Choosing to exempt your student from required vaccines
If your child is not exempt from any of the required vaccines, or if your existing exemption form does not show that your child is exempt from all required vaccines, you must either:
· Complete the online education module at https://immunize.utah.gov/immunization-education-module/ and ensure you select all vaccines you wish to exempt your child from,
· Visit your local health department for an in-person consultation to receive an exemption form for your child, or
· Provide the school with written proof of vaccination for your child.
Please submit a copy of the exemption form and/or immunization record to the school by the first day of school.
Message from the Utah Health Department (as of July 2025)
We have recently had several measles cases reported in our state, and we want to encourage you to check your child’s immunization records, specifically for the MMR vaccine. Please make sure your child has received two doses of the MMR vaccine to remain protected in case of potential exposure in your community or at school.
