- ELA Level 5 or higher
- Native English Speakers: ABE 2 or higher
Step 2: Complete the CNA Intake Application
The CNA Intake Application helps us learn more about your educational and career goals and allows us to determine your next steps.

Step 3: Meet with a Proxima Navigator
A Proxima Navigator can help you:
- Explore your education and career options
- Understand the CNA program requirements
- Answer questions about training and enrollment
- Connect with available resources and support

Step 4: Complete the CNA & Proxima Participation Form
Before enrolling, you will complete a brief form confirming that you:
- Are interested in completing CNA training
- Are committed to attending classes and successfully finishing the program
- Understand program expectations
- Agree to pay a $50 refundable deposit
Enrollment Checklist
✅ Complete the Granite Peaks Intake Form
✅ Take your placement test (if required)
✅ Meet with a Proxima Navigator
✅ Complete your final enrollment paperwork
