How to Get Started

Interested in becoming a Certified Nursing Assistant (CNA) through Proxima?

Follow the steps below to begin your path toward a rewarding healthcare career.

CNA Program Information

Minimum Requirements:

  • ELA Level 5 or higher
  • Native English Speakers: ABE 2 or higher

Step 2: Complete the CNA Intake Application

The CNA Intake Application helps us learn more about your educational and career goals and allows us to determine your next steps.

Step 3: Meet with a Proxima Navigator

A Proxima Navigator can help you:

  • Explore your education and career options
  • Understand the CNA program requirements
  • Answer questions about training and enrollment
  • Connect with available resources and support

Visit our Start Your Journey page to complete the Proxima Interest Form and schedule a meeting.

Step 4: Complete the CNA & Proxima Participation Form

Before enrolling, you will complete a brief form confirming that you:

  • Are interested in completing CNA training
  • Are committed to attending classes and successfully finishing the program
  • Understand program expectations
  • Agree to pay a $50 refundable deposit

Enrollment Checklist

✅ Complete the Granite Peaks Intake Form
✅ Take your placement test (if required)
✅ Meet with a Proxima Navigator
✅ Complete your final enrollment paperwork
