Youth-in-Care (YIC) Education Services at Granite School District provides educational support and interventions for individuals under the age of 21 who are:
- In the custody of, or receiving services from, the Utah Department of Health and Human Services (DHHS) or an equivalent Native American tribe, or
- Placed in a juvenile detention facility.
Our team delivers educational, administrative, and wraparound support services to eligible students. We are committed to ensuring equitable access to high-quality educational opportunities that help Youth-in-Care grow academically, socially, and emotionally, preparing them for future success.
This page outlines the registration process and required information for enrolling a student in state care into Granite School District.
- Students in the Juvenile Justice System who are assigned to a secure facility or program will be registered onsite into Granite School District’s YESS School.
- All other Youth-in-Care students should follow the registration protocol outlined below.
The following documentation is required for student placement. Please email the below information to Erin Gubler at ekgubler@graniteschools.org:
- Immunization Records
- YIC Form (PDF) (Please fill in all applicable information)
- Copy of Birth Certificate
- Custodial Involvement History
- Special Education IEP (if applicable)
- Education Records – Student Transcript and/or Report Card
- Psychological Report (if applicable)
- Safe School Violations
- BIP and/or FUBA Plan Documents
- 504 Documents
If a student has an IEP, the YIC form and IEP will be forwarded to the Special Education team to determine proper placement.
Please note that all students must be properly withdrawn from their previous school. We ask that you notify the student’s mentor and/or YIC office when there is a change in caseworker, case manager, or foster/proctor parent and address.

For Agencies needing documentation for licensure, the Youth Education Coordinating Form(PDF) is a document used by Health and Human Services and Granite School District to establish an educational plan for youth in custody, outlining the services to be provided, the entity responsible for delivery (school district or other accredited provider), and the funding plan. The form requires information about the human services program, the education service provider, and details about the youth and their specific educational needs to ensure they receive appropriate and comprehensive schooling.
