Ameriben, Blue Cross, Direct Care Administrators, DMBA,
Educators Mutual, GEHA, Meritain, PEHP, SelectHealth,
UMR, United Health Care, University of Utah, CHIP,
Medicaid for children, Medicare part B, and some
Medicare Advantage Plans.
