Student Vaccinations
Our local high schools will be hosting COVID-19 Vaccination clinics on behalf of the Salt Lake County Health Department. This clinic will be for all students 12 years of age and older on the following dates:
Cottonwood High School, Monday, May 24th from 8:00 am – 11:00 am
Granger High School Tuesday, May 25th from 8:00 am – 11:00 am
- In order to receive a vaccination, each student will have to complete a registration form and have a consent form signed by a parent/guardian. Your child is NOT required to be vaccinated and your explicit permission is required. You can get required forms at the high school you are receiving your vaccination.
- The registration form asked for insurance information. If you have insurance, please fill in the information needed. If you do not have insurance, leave that portion of the registration form blank. It will not affect your student’s ability to receive the vaccination.
- Students are asked to wear short-sleeved shirts, for ease of vaccination.
- There will be an observation area for students after receiving their vaccinations. They will be required to be monitored for 15 minutes before they will be allowed leave.
- Parents are required to accompany their student while they are being vaccinated. If a parent or family member would like to receive the vaccination themselves, they can complete the registration form, show their ID and receive the vaccination at our school. The health department has authorized that anyone 12 years of age or older may receive the vaccine, including the second vaccine shot at our school.
- It is best to eat and drink before receiving the vaccine. This will help reduce the risk of experiencing any symptoms such as dizziness, light headedness, or fainting.
